Member Journey
Comprehensive Intake
During this comprehensive assessment, we will delve deep into your complete health history, lifestyle, and aspirations. We will also order in-depth lab work for you to complete.
Personalized Care Plan
After reviewing your lab results and the information we gathered at your intake, we will formulate a personalized, integrated care plan. Whether you’re trying to get to the root cause of a problem, or seeking a way to optimize your health, this actionable plan will include everything you need to get there.
On-Going Check-Ins
We will individualize a follow-up schedule (quarterly at a minimum) to assess your progress, troubleshoot barriers, discuss any new concerns, and modify your care plan accordingly.
Sick Care
As your primary care provider, we will be there for you for all of your primary care needs, including sick care, referral coordination, and ensuring you are up-to-date on all relevant screenings.

Chandler Fogarty, NP
Chandler is a board-certified Family Nurse Practitioner specializing in functional medicine. She earned her Master of Science in Nursing from Duke University and is in the process of obtaining her Functional Medicine certification. Her diverse clinical background ranges from her early experience as a Cardiac Surgery ICU Registered Nurse to her work in medical aesthetics and now primary care.
Guided by science and centered around each patient’s unique biology and goals, Chandler created Elysium Health to deliver primary care the way it should be: focused on restoring balance, enhancing resilience, and creating vibrant, thriving lives.

Does Elysium Accept Insurance?
No, Elysium does not accept insurance for visits. With this model, we are able to provide our members with a high-touch, integrated, and personalized care model.
Medications and follow-up lab work will be billed through your insurance.
Do I Have to Live in MD to Join Elysium?
Yes, Elysium is only able to care for patients in MD at this time. We are working to expand our care to other states, though - stay tuned!
How Much is Membership?
Our membership fee is $180/month ($2,160/year). We require a 12-month commitment for members.
What's Included in Elysium Membership?
Your 12-month Elysium membership includes the following:
